If you choose to register with YourQuest Personal Health Solutions, LLC© by completing a personal profile ("My Profile"), the registration process of My Profile and subsequent assessments ("Health Risk Assessment") will request certain personal information, including, but not limited to, individually identifiable health information ("Protected Health Information"). YourQuest Personal Health Solutions, LLC© will gather the Protected Health Information that you voluntarily provide in My Profile, the Health Risk Assessment, and in other interactive features within the Site, and will use such Protected Health Information to communicate with you and to personalize your online experience with the Site. Protected Health Information may include sensitive information relating to diseases, psychological or psychiatric treatment, behavioral or mental health services, or alcohol abuse. YourQuest Personal Health Solutions, LLC© will keep confidential any Protected Health Information you provide as outlined herein. YourQuest Personal Health Solutions, LLC© does not collect, and the Site will not permit the collection of, Protected Health Information from any person of which YourQuest Personal Health Solutions, LLC© has actual knowledge is a child under the age of 13.

By your acceptance of these Terms of Use, you hereby authorized YourQuest Personal Health Solutions, LLC©, without identifying you individually, to disclose to the Company or to our business partners data or reports that have been aggregated from Protected Health Information provided by you and by other authorized Company users of the Site.

In addition, by your acceptance of these Terms of Use, you hereby authorize YourQuest Personal Health Solutions, LLC© to use or disclose, but only as set forth below, your applicable Protected Health Information in the following instances:

• You give your advance consent to the disclosure of your Protected Health Information to a specific person or entity;
• YourQuest Personal Health Solutions, LLC© needs to share the Protected Health Information with a person or entity to enable such person or entity to provide the service or product you have specifically requested;

By providing your Protected Health Information on the Site (or in an e-mail), and through your acceptance of these Terms of Use, you are deemed to express your consent as set forth above and are the equivalent of your signature evidencing your consent.

Your Protected Health Information is subject to these Terms of Use. You may obtain a written copy of these Terms of Use by using the "Print" function on your screen. Upon your consent to the use or disclosure of your Protected Health Information as noted above, such Protected Health Information may no longer be protected by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") or state privacy laws. You may revoke your consent to the continued use or disclosure of your Protected Health Information (except in the case of a governmental or legal proceeding) at any time by notifying YourQuest Personal Health Solutions, LLC© in writing as set forth below, but if you do, the revocation will not have any effect on any actions YourQuest Personal Health Solutions, LLC© or the Company took before the receipt of the revocation of your consent. You are entitled to view and copy the Protected Health Information you have provided to the Site. While you decide what Protected Health Information to provide, if you choose not to provide certain information, or if you do not consent to the disclosure of Protected Health Information when requested, this action may limit access to some features of the Site. The Site provides a means for each person registering to correct, update or delete any previously entered Protected Health Information.

YourQuest Personal Health Solutions, LLC© automatically gathers typically anonymous technical information about each visitor, such as Internet domain, IP address, browser type, operating system, pages visited, the URL of any site used to link to the Site, and other related information.
